I find that some stations (since they are franchises after all) are lax in the maintenance department. All fuel pumps have filters on them (whether you can see them or not) and I have seen external ones leaking because they are so plugged that fuel is bypassing the o-ring. In an emergency the inside small o-ring can be removed, essentially bypassing the filter entirely (I learned that trick from my trucking days). I would not put it past "scabby" dealers to do that to save money on filters and labor.
Another note. I found in Montana (at Exxon and some Sinclair stations) that premium would not have ethanol in it, even though mid-grade and regular would. That alone determined where I would fill.
