Clutch basket plug keeps falling out

Hello everyone! This morning on my commute to work, I was getting on gas in 3rd gear. At about 9k rpms I hear and feel a big clunk as a I shift into 4th. I quickly pull in my clutch and quickly thought the plug in the basket fell out, AGAIN! I shifted up again and it felt fine and went thru the gears nothing felt odd. I get to a stop light and sure enough I'm having a hell of time getting it into neutral. This is not the first time this has happened. It fell out about 3 months ago, and I thought I had popped it on good. Any tips on getting that sucker to stay?

Last afternoon I got around to putting the plug back where it goes. Ended up using the female end my long 1/4 extension to tap it back in with a mallet. Lets see if the sucker doesn't pop out again. One photo is the before and after
